CGI form script

I need a cgi script written to handle the form currently located at:

http://www.prolaw.co.uk/register.htm

I need all the information emailed to the client with a designated file attached. Then the browser should display a thank you page which I will design. The site is to be hosted on a Window NT server.

If anyone could get me a quote that would be fantastic,
