Fix our form, teach us how

We have a small brochure-style website with 2 basic forms which we built using FormMail. We are having intermittent faults and need you to fix them (see http://www.leedsanimation.org.uk/order.html). We would also want you to show us what went wrong.
We also want to create another simple feedback form. Ideally, you would come to our office and teach us how to create it. The person you would teach has rudimentary scripting knowledge (i.e. can mess about in FormMail) so you wouldn't have to start entirely from scratch.
Please give us a two prices, one for fixing the existing form, and one spending a half-day teaching us to use FormMail or its equivalent effectively. Please be located near enough to Leeds that you can come to our office.
