You've just posted some HTML that'll never do anything other than client
side stuff.
You need backend code to attach the CV. Upload it to the server base64
encode it then write it onto the email using the right headers
Get a new ISP, possibly one that supports PHP and most will support Perl
CGI's then I'm sure that you could find Open source scripts to do what
you want
If not, I can do you a PHP or Coldfusion one in a few hours
