FN-FORUM: online form with file attachment facility

i have a client who wishes to have an online recruitment form on their site
(hosted on windows 2000 server).

this needs to send standard form details and attached CVs to a specified
email address.

i can do the basic details bit but does anyone know how to do the attached
file bit? apparently the server has SAFileUp installed. will this help?

a step-by-step guide would be helpful. or can someone set this up (quotes
off-list direct to me please! [EMAIL REMOVED]
