Although I can't find any decent information on it, I'd say it's almost
certainly VoIP. If you look at the system overview under 'How does it
work?', you'll note the 'broadband telephone adapter' plugs directly into a
hub and travels over the 'Internet' (rather than ATM) to a voice switch. It
doesn't say anything about needing other protocols.

Not off-hand. The expensive bit would be the switch on the far end
(Internet -> PSTN), which no doubt BT could get much cheaper than everyone
else, so...
