I found the SAMS "PHP and MySQL Web Development" book to be excellent.
It starts with the basics and then builds up to some pretty complex
examples. My copy is pretty out of date now, though you can pick up a
copy on Amazon for a fiver (not bad for 850 pages!) - the ISBN is
0-672-31784-2. There may be a newer version out now though.
