Build up a list of potential firms / contacts
Phone them up, find out the specific person you need to talk to, in my case
nearly always the creative director. Speak to them, briefly introduce
yourself and what you do - appreciate that they are very busy and may not
want to talk to you. Ask if it is ok if you periodically send an email with
your latest work in it. If the person you need to talk to is not available /
can't talk to you see if you can send an email anyway.
Send out a mail every month or so to your list.
Worked for me - took about 3 months to build up, now I am busy all the time
and don't have to do any sales. Like a few people are suggesting I had to do
a couple of projects for free to build up my portfolio before I did the
initial contact thing.

going from free to paid is impossible - never work for someone else
for nothing - if you need a portfolio create stuff for you....
