If you're inclined to have the screen interface in flash, mdm's zinc
has database extensions that allow read/write to a local database
including access & mysql. INM V12 database which has done similar...

I would say import the form into a table, hence controlling the layout
within the iframe, and make sure it scrolls then no matter how long the form
gets it will still fit.
