Have you though about getting them to sign off the designs?
I always get my clients to sign a piece of paper approving the
designs before I start the build, and explain to them that if they
make any changes to the look-and-feel after this stage then it won't
be cover by the original quote and will cost them extra.
That way when a client comes back (which they're less likely to
having signed something) and changes their mind I can show them the
piece of paper and say "it's up to you, I'm happy to change the
design but it will cost you extra". They usually opt for the cheaper
option.
