I think you are right, its something to do with the way PHP is set up
on Mac OS, but I've Googled and haven't been able to find out how to
force it to give a value to $PHP_SELF.

I'm quite keen not to have to change any actual code between the
testing server and the production server if I can possibly avoid it.

I don't think so. I think its valid to leave out the semicolon when
using a single variable like this. Well, I've been doing it this way
for years, and as I say, it works on the production server
