FN-FORUM: regions by country

I'm looking for a detailed list of "regions" by country, preferably sql
format , but csv / xml is also fine as defined below, if I can get the
raw data, I'll happily munge it.

the final data I need will look like this,.

id country_id region_id country_name region_name
1 UK xx United Kingdom Staffordshire
...
300 US xx USA Alaska
..
600 AU xx Australia New
South Wales

Obviously I don't expect every country and every region. and I
appreciate that some regions may be by convention rather than actual
regions

I do have a small budget for this (bit not enough for the iso fees,
which is why im asking here) if you already have what I need, otherwise
if you all send me what you have off list, I'll munge it and put it on
the wiki for general use


(oh and thanks for the answers to my last questions - I'm not one for
generating list traffic just to say thank you to a question that only I
really care about , but it is appreciated)
