Ken,

What browser are you using, with Firefox I get a different page than
with IE, the FF one looking more like you have to have a PP account to
proceed.

However, there is still a button on this page which says...

If you do not currently have a PayPal account [Click Here]

Also, things appear differently dependent on how you set the option for
whether you need delivery.

On IE if you say no delivery is required, it will go to a one page
screen which asks for CC details straight away. If you say delivery is
required it's a two page jobby!
