I use Jaws (covermount on one mag or another every month usually)
you could try:
http://www.cutepdf.com/Products/CutePDF/writer.asp
(which needs Ghostscript - as most of the free ones do)
or if you have Adobe products, you might have PDF Writer lurking somewhere
(I think it came with elements - a simple print to pdf...
