Managed (i.e. they install/upgrade software and deal with any problems) or
unmanaged (i.e. you do everything that isn't hardware)?

What levels of reliability, in case of hardware failure? OK for the server
to perhaps die for a day? or just an hour? or more reliable than that?
What levels of support for software problems?

There is a vast range of different hosting, depending on your budget and
your requirements :)
