I did one for a site I hadn't designed myself. It integrated quite well
and does pretty much what you want to do. Have a look at

Like I say, I didn't design the site, but provided them with the booking
code. It does no card payments. The client wanted to contact any
customers prior to booking and deal with payments off the site.
